ADULT HEALTH SERVICES / HOME HEALTH CARE - Services provided in your home to maintain & improve health and safety, on an intermittent, part time basis by licensed & certified Public Health Nurses/Registered Nurses, certified home Health Aides and trained Homemakers

Public Health Nurses in Adult Health/Home Health CareSkilled Nursing Services - Public Health Nurses assess health needs and provide services to help recover from illness or injury or to adjust to and live with chronic or acute illness or injury
- Assess health needs and plan for care
- Communicate with family physician about changes in health status
- Direct nursing care including dressing changes, medication administration, monitoring of blood pressure, weight, pulse & respiratory status, and medication setups
- Observation for changes in health status
- Support to individuals and families
- Coordination of services with other community organizations
- Case management - care coordination
Home Health Aide Services - Services provided under the direction and supervision of a Public Health Nurse
- Personal cares such as bathing, grooming, dressing
- Activities of daily living
- Exercises
Homemaker Services - Provided under the direction & supervision of a Public Health Nurse
- Light housekeeping tasks
- Laundry
- Grocery shopping and errands
- Meal preparation
Long Term Care Consultation
- An assessment done in a person's home by a Public Health nurse and Social Worker, to determine a person's health and social needs and preferences, and the best plan for care to meet those needs, prior to nursing home admission
- The screening is an excellent resource for families not sure of what to do to continue care for a loved one, and is a valuable source of information about supportive services available in the community
Alternative Care Services
- If the LTCC team finds that a person is able to remain at home and chooses to do so, community services will be offered and care arranged to help one live as independently as possible. If eligible, home care services may be paid for by Alternative Care (AC), Elderly Waiver (EW), or Community Alternatives for Disabled Individuals (CADI) programs
South Country Health Alliance - Community Resource Management Team (CRMT)
- Special assistance and care coordination services by a registered nurse and/or social worker, for persons eligible for Medical Assistance through South Country Health Alliance, a county-based purchasing plan, covering individual health care services
Payment for Services: Public Health will bill the following payers for payment as appropriate:
- Medicare
- Medical Assistance
- South Country Health Alliance
- Veteran's Administration
- Health Insurance's
- Alternative Care/Elderly Waiver
- Long Term Care Insurance
- Private pay - fees are adjusted according to income and assets
How do I arrange for services?
- Persons may request services for themselves or others
- Physicians, hospitals, clinics, professional staff and other community agencies may refer persons for services
