Department Head Evaluation
      
Purpose:  The purpose of establishing a uniform performance appraisal process is to ensure that all employees are treated fairly.  The goal of Brown County is to enhance the quality of work that each employee has to offer.  The performance appraisal process is intended to be a positive experience and should improve communication, trust, and public service delivery. 
 
Definition of Ratings:
3 - Exceeds Expectations:  Consistently exceeds all relevant performance standards and far exceeds job requirements.
2 - Meets Expectations:  Performance fully meets job requirements. Familiar with the knowledge and skills required for the job.  Errors are within acceptable limits, in terms of frequency and consequence.
1 - Below Expectations:  Performance meets some, but not all, job requirements.  A written plan of goals and a specific time frame must be included.
0 - Deficient:  Consistently falls short of performance standards.  A Performance Improvement Plan must be made and followed for the employee to improve performance.

Note: Annual salary step increases, for Department Heads on the Salary Administration Plan, are contingent upon job performance as determined through the performance appraisal process. Steps are awarded unless any one (1) category is marked as “deficient”, two (2) or more categories are marked as “below expectations” or if the overall average performance rating is under 2.00.
 
Instructions:  Describe the employee's (or your own) contributions and professional strengths or weaknesses in each of the categories listed. Illustrate specific, detailed examples of goals, results and job related behavior since the last review. Ratings must support and be substantiated by narrative comments.
